Our Evaluation Framework
The HAP Clinic Evaluation Framework
Developed with reference to the Quadruple Aim Framework, and an extensive Scoping Review conducted by the HAP Research Team, this Evaluation Framework will be used to inform the evaluation of the HAP Clinic.
The HAP Evaluation Framework has 4 objectives:
- Patient Experience
- Health of Populations
- Systems of Care Delivery
- Provider Experience
In order to collect information for the four evaluative objectives, Research Team will collecting impact stories, metrics and data from the HAP clinic, and this will further aid in the clinics development, effectiveness and sustainability.
