The HAP model emphasizes the importance of community partnerships to ensure that the communities we serve have appropriate and accessible healthcare, informed by a community needs assessment conducted by Humber Polytechnic nursing students.
St. Francis Table is a restaurant serving individuals facing food insecurity in Toronto’s Parkdale community. Operating six days a week, it provides an average of 250 meals daily, primarily to seniors and unhoused individuals. More than just a meal, it fosters community, advocacy, and connection.
