The Health for All Partnership (HAP) is a collection of nursing-student led, faculty supervised clinics that, in collaboration with our community partners, provide health services to equity deserving communities in the Greater Toronto Area (GTA).
Grounded in a relational nursing lens and evidence-informed practice, the HAP is designed to offer services tailored to the specific needs of each community.
Our goal is to provide innovative and sustainable healthcare to our communities while empowering nursing students to apply their scope of practice in creative ways.
This project seeks to develop a nurse-led health partnership model that connects healthcare with social services to provide integrated health services to communities and people with complex health needs who experience barriers to care access.
As we recognize needs vary across sites, the HAP clinic provides holistic healthcare services including primary health care, health education, mental health support and advocacy.
The HAP Model of Care is guided by a community needs assessment conducted by nursing students in the early stages of the project, to ensure that services reflect the true needs of the community – because, ultimately, the community is the expert. This model enables the clinic to provide a broad range of services that address the social, physical, psychological and socioeconomical aspects of health to support each client’s well-being. These core principles are threaded into our healthcare delivery across all sites.
Our services include primary care, allied health triage, and referrals when necessary. Nursing students at the clinic provide wound care, chronic disease education, and health screenings such as blood pressure checks, blood glucose monitoring, and urinalysis. They also offer general health assessments, including vital signs and height and weight measurements. The clinic is equipped with supplies for wound care, and pregnancy testing.
The clinic offers health education on a wide range of topics, including—but not limited to—dental hygiene, mother-baby care, nutrition (including budgeting and meal preparation), understanding and accessing health services, and sexual health.
We recognize the vital role mental health plays in overall well-being. We offer a social support hub where clients can access a variety of resources and services to meet their social and emotional needs. The clinic is a safe, welcoming space for all members of the community. We offer drop-in coffee chat sessions providing opportunities for connection, reflection, and support in a relaxed, inclusive environment.
We recognize that accessing the right resources can be challenging for some individuals. That’s why the HAP Clinic hosts seasonal clothing drives to support clients during both colder and warmer months. We also provide essential items such as health hygiene packs, coats, non-perishable food (including canned goods), and feminine hygiene products. In addition, we can connect you to other health services and offer referrals to programs tailored to your specific needs.
